About Heal Functional Med

What is Heal Functional Med?

Heal Functional Med is a direct primary care and functional medicine practice at 121 McNary Heights Drive N. in Keizer, Oregon. Dr. Allyson Howarth, DNP, FNP-C, MPH, WHNP, is the founder and CEO and the prescriber, and she has been a nurse practitioner since 2016. Its membership covers most primary care services, and functional medicine work sits alongside it: hormone health, bioidentical hormone replacement, peptide therapy, functional lab testing, nutrition, gut health, environmental medicine and lifestyle medicine.

It does not list a weight loss program, a GLP-1 service or a longevity medicine service by name. It names no medications.

Office hours are Monday through Thursday, 9:00am to 4:00pm, Friday 9:00am to 12:00pm, and closed on weekends. The practice serves Keizer and surrounding areas, including Salem, Silverton, Dallas, Albany, Woodburn and Portland. Its Google rating is 5 from 2 reviews. On contact between visits, it says: "As a member you can call, text, even video-call your physician with your medical and health questions." You can reach the office at (503) 744-6230.

What does it cost?

Membership starts with a one-time enrollment fee of $425. A $125 deposit secures your first visit, is non-refundable and is applied to your balance. After that, membership is charged monthly to a card on file: $100 for an individual, $180 for a couple in the same household and $300 for a family of up to five. Families over five are asked to call for pricing.

A cash-based option carries the same $425 enrollment fee, with follow-up visits at $250 or $350 each, depending on length. It does not state a consultation fee apart from the enrollment fee. It does not publish prices for weight loss, hormone therapy or peptide therapy, and it does not say whether medication is included in the monthly fee. Prices are subject to change, so it asks patients to call for official pricing. It does not bill Medicare, and it does not say whether other insurance is accepted. Membership carries a 6 month minimum before cancellation, and cancellation requires 30 days notice.

What should you ask before you book?

Ask whether the $100, $180 or $300 monthly fee covers any prescriptions. Medications are paid at the time of service or added to the monthly fee when they apply, so ask which applies to hormone or peptide care. Ask whether hormone medication is brand-name or compounded, since it does not say, and what a typical month of it costs. For peptide therapy, ask which peptides would be used, because it names none, and what the full program costs. Ask what labs, imaging and follow-up visits cost after the membership discount. Ask whether it accepts any insurance besides Medicare, and whether you would still need your own plan. Finally, ask how cancellation works once the 6 month minimum has passed.

Who is it a good fit for?

This is a good fit if you want one prescriber for ongoing primary care alongside hormone or peptide care, can commit to at least 6 months of membership, and are 16 or older. It does not accept patients under 16 and does not provide narcotic pain management, so those needs call for a different practice.
