What is Revati Wellness?
Revati Wellness is a concierge primary care practice at 29001 Cedar Rd, Suite 655, in Lyndhurst, Ohio. It serves patients in Lyndhurst and the greater Cleveland area. Dr. Tom Morledge is the physician who prescribes, and he founded the practice in 2011. He is board certified in internal medicine. He trained at MetroHealth, then spent fourteen years as medical director of the Cleveland Clinic's Willoughby Hills Family Health Center.
The practice is limited to under 300 patients. Members get 24/7 communication with their doctor, and the practice has access to the electronic medical records of Cleveland Clinic and University Hospitals. Its six services are concierge medicine, functional medicine, hormone replacement therapy, lifestyle solutions, heart health and prevention, and whole body MRI. Hormone therapy is one of those six, not the whole practice. Weight loss medication and peptide therapy are not among its services. Hours are Monday through Thursday, 9 am to 5 pm, and Friday, 9 am to 1:00 pm, with more available by appointment. Google lists the practice at 4.9 from 32 reviews.
What does it cost?
Concierge membership is $460 a month, with a minimum one-year commitment. Concierge membership with Whole Body MRI is $710 a month. It does not publish a separate price for hormone therapy, and it does not say whether the monthly fee covers the hormone medication, lab work or follow-up visits. Insurance acceptance is not stated.
The first step is a complimentary Meet the Doc consultation. It is a 45-minute, non-medical meeting. If you decide to join, you sign a membership agreement, an enrollment form and a release of medical records. Cancellation terms are not stated.
In its own words: "Before joining, visit our office, meet the doctor, and ensure this is the right choice for you."
What should you ask before you book?
Does the $460 monthly fee include the hormone medication, the lab work and the follow-up visits, or are those billed on top? Is the medication brand-name or compounded, and which pharmacy fills it? Ask whether Dr. Morledge writes every hormone prescription himself. How often are blood tests drawn, and what do they cost? Can follow-up happen by phone or video, or only in person? Does the practice take insurance at all? If you leave before the year ends, is the rest of the membership still owed?
Who is it a good fit for?
This is a good fit if you want concierge primary care in Lyndhurst, can commit to at least a year at $460 a month, and want hormone therapy managed within one ongoing relationship with a physician. If weight loss or peptide therapy is your main goal, this practice does not offer either.

We have not yet collected published prices or prescriber credentials for this clinic. When we do, they will appear here with a link to the page they came from — or a note saying the clinic publishes none, which is itself worth knowing. Call them for a price in the meantime.
