What is Erlacher Medical Services?
Erlacher Medical Services is a Direct Primary Care practice at 130 Roundabout Ct in Rocky Mount, NC 27804. Its main work is primary care for adults and children, paid through a monthly membership instead of insurance. Gray Erlacher, MD, is the only prescriber the practice names. The clinic calls itself a new practice, and Google lists it under Doctor with a 5 rating from 11 reviews.
Hormone therapy is the clearest of the services. The practice offers bioidentical hormone replacement therapy (BHRT) for menopause, andropause and other hormone deficiencies, using topical hormones, hormone pellets, injections and oral supplements. It says Dr. Erlacher was among the first physicians in the country to adopt BHRT for menopause and anti-aging in the 1990s. For weight loss, it names Ozempic and Wegovy, but only in the context of diabetes management. It does not describe a weight loss program, and peptide therapy is not listed as a service.
What does it cost?
The membership is $50 a month per member, with a three-month minimum commitment and cancellation anytime after that. Each office visit carries a $20 co-pay, with no limit on visits. The $70 on the homepage is that membership plus one co-pay. Prescriptions, labs and X-rays are still covered by your insurance at the pharmacy, lab and imaging center, but the membership itself is not covered by insurance. In its own words: "We are straight up with this, no gimmicks or hidden fees"
Cosmetic medicine, bioidentical hormone therapy, IV infusions, acupuncture and minor surgical procedures are priced separately from the membership. It does not publish prices for hormone therapy or weight loss. It does not say whether the monthly fee covers any medication.
What should you ask before you book?
Ask who signs your hormone prescription, and whether Gray Erlacher, MD, handles the hormone visits and follow-ups. Ask whether the hormone products are brand-name or compounded, since a blog post describes a compounded vaginal hormone cream. Ask whether the monthly fee includes any medication, and what the medication itself costs. Ask what labs, pellet implants and follow-up visits cost.
If weight loss is your goal, ask whether Ozempic or Wegovy would be prescribed for that purpose and what it would cost. Ask whether peptide therapy is offered, since a blog post mentions PT-141 but the services list does not. Ask for office hours and whether any visits are by telehealth, since neither is published. Finally, ask how cancellation works once the three months are up.
Who is it a good fit for?
This is a good fit if you want a membership doctor you can reach quickly and you are ready to pay out of pocket for hormone therapy or add-on services once you have the prices in writing. It is a weaker fit if you need published prices, posted hours or a structured weight loss program before you commit.

We have not yet collected published prices or prescriber credentials for this clinic. When we do, they will appear here with a link to the page they came from — or a note saying the clinic publishes none, which is itself worth knowing. Call them for a price in the meantime.
