Start with who signs the hormone prescription, because no one is named for that service. Then ask whether the price includes the pellets, the lab work before treatment, and the follow up labs. Its pellets are BioTE, so ask which product is placed and whether any part of your plan is compounded, meaning mixed by a pharmacy rather than made by the manufacturer.
For ketamine or Spravato, ask what a single visit costs, how many visits a course usually involves, and whether insurance will cover them. Ask how cancellation and rescheduling work, and whether a fee applies. If you want follow up by video, ask that directly.
