About Larkspur Integrative Health

What is Larkspur Integrative Health?

Larkspur Integrative Health is a cash-pay clinic at 150 E Riverside Dr. Suite 100 in Eagle, Idaho, inside Rose Glow. Melanie Maltese, APRN, FNP-C, MSN is the nurse practitioner who prescribes and directs care. Its main work is hormone care: bioidentical hormone therapy for perimenopause and menopause, and testosterone therapy under a membership. It also offers peptide therapy, postpartum, metabolic, gut, vitamin and thyroid care. The two medications it names are the peptides Sermorelin and GHK-Cu. Telehealth visits are open to patients throughout Idaho. It is rated 5 on Google from 7 reviews. No office hours or opening year are given.

What does it cost?

Visits are priced one at a time. A 60-minute Foundational Deep-Dive for new patients is $250. A 45-minute lab review is $175, a 30-minute follow-up is $125, and a 15-minute check-in is $75. Vitamin injections run $20 to $50.

The TRT membership is $199 a month and includes visits, labs, medication and at-home delivery. The peptide membership is $299 a month, a founding rate limited to the first 20 patients. In its own words: "All pricing is provided upfront so you can invest in your health with confidence and clarity." Outside the memberships, labs and medications are separate costs, and no prices are given for them. It does not bill insurance but will provide a superbill, and HSA and FSA cards can typically be used. No cancellation terms are published.

What should you ask before you book?

Ask what the peptide membership covers. Only Sermorelin and GHK-Cu are named, and the specific peptide, dose and schedule are set at the first consultation, so ask for the full list and confirm which medication costs fall inside the $299 rate. Ask whether the medications are compounded or brand-name. Many of its peptides are off-label and not yet FDA-approved, so ask what that means for you and what consent involves.

Ask what labs and follow-up visits cost between memberships, whether your first visit must be in person, how to cancel, and whether the founding rate is still open. Ask who you will see at each visit, and whether Melanie Maltese reviews and signs your prescriptions herself.

Who is it a good fit for?

This is a good fit if you live in Idaho, can pay cash each month, and want one named nurse practitioner overseeing hormone or peptide care. It is a weaker fit if you need insurance billing or want every lab and drug priced before you start.
