Lotus Elite Primary Care and Remote Health Services, also called Lotus Elite Care, is a membership-based primary care practice at 24 Old Kings Rd. N in Palm Coast, Florida. What does it cost?

Memberships are paid monthly. The Access Plan is $15 a month and includes telehealth access, with telehealth visits at $45 each. The All Access Plan is $25 a month and adds home visits at $100 each. The Primary Care Plus Plan is $120 a month, with telehealth included and home visits at $100 each as needed. A $250 a month plan covers all primary care needs, including annual physical and labs, with telehealth and home visits at no additional charge. The Men's Health Plan is $180 a month, with telehealth and quarterly home lab visits included and extra home visits at $100 each. It does not say whether testosterone medication is included in the $180.

Outside the memberships, injectables are $10 per unit, or $8 per unit for a party of four or more. Versa filler is $450 a syringe, or $400 with a party of four or more. Sports physicals are $25 with no membership needed. Visits between 10pm and 7am carry an after hours charge of $50 for telehealth and $100 for home visits. Card payments carry a 4% processing fee. It accepts HSA payment and can provide a superbill for submission to insurance, but it does not guarantee reimbursement. Cancellation requires 30 days notice. It does not publish a consultation fee.

What should you ask before you book?

Ask whether the $180 Men's Health Plan includes the testosterone medication, and if not, what the medication costs on top of the monthly fee. Ask whether the drug is brand name or compounded, and who signs the prescription, since Hannah Magnuson has no credential abbreviation published.

Ask which contract applies. One part of the clinic's materials says there are no long term contracts, while its FAQ says members sign an annual contract. Ask whether you are still charged during the 30 days after you give notice. Ask what follow up visits and lab work cost beyond the plan, and whether there is a consultation fee.

Ask which states its telehealth service covers, since none are listed. Finally, ask whether a home visit will be available when you need one. The FAQ says home visits may not always be available, even though members can reach a provider at all times.

Who is it a good fit for?

This is a good fit if you live in or near Flagler or Volusia County, want a monthly membership with care by telehealth or at home, and will get the medication cost and contract terms in writing before you pay. If you are looking for a medical weight loss program, this clinic does not list one.
