## Who is the physician?
Allan Kurtz, D.O. is the only provider named. His bio page publishes several further lines: board certified in internal medicine, board certified by the American Osteopathic Board of Internal Medicine, a fellowship at the University of South Florida with the American Board of Anti-Aging Regenerative Medicine, medical director of this center, and medical director of Warner Medical Center. The bio gives date ranges for those directorships, 2009 to present here and 1987 to present at Warner, but the site never states a number of years and we have not done that arithmetic. No second provider is named, and there is no FAQ or patient-agreement page.
## Where is it, and what else is missing?

Business hours are not published; the contact page gives only an office phone and a fax number. No telehealth or virtual-visit offering is described, and no states are listed. Nothing on the site states who the care is for or how a new patient's process runs. One more thing to flag: the business name renders three different ways across the site, adding "Regenerative" on the homepage and "Functional" on the contact page.
