About Fishtown Medicine

What is Fishtown Medicine?

Fishtown Medicine is a direct primary care practice in Philadelphia, at 2418 E York St. It is run by one physician, Ashvin Vijayakumar, who goes by Dr. Ash and is Board Certified in Internal Medicine. It works as a membership. Members get visits, texting and calls with Dr. Ash, and he makes house calls within its Philadelphia neighborhood footprint. Follow ups and lab reviews happen by text, phone or secure video, and it offers virtual care in 40 states.

Primary care is the main business, from sick visits and refills to physicals. On top of that it covers three of the four areas a cash pay patient tends to ask about: hormones (TRT, thyroid, perimenopause, menopause and PCOS), metabolic health and weight (GLP-1s, advanced lipid testing and glucose monitoring), and longevity and prevention, including VO2 max and DEXA testing. Visits run 60 to 90 minutes. The practice opened in 2024.

What does it cost?

It does not publish a separate price for GLP-1 care, hormone therapy or longevity work. Care comes through a membership, offered monthly, quarterly or annually, and its stated price range runs from $195 to $5,000 without a breakdown by plan. There is a free 20 minute consult before you commit. The membership covers visits with no copays or visit fees, and it is HSA/FSA eligible. Insurance is kept for labs, imaging and prescriptions, so medication costs run through your own plan.

What should you ask before you book?

Ask what the membership costs at each level and what each one includes. If you want a GLP-1 or testosterone, ask whether the drug goes through your insurance and what you pay if insurance declines it. Ask which tests, such as DEXA or VO2 max, are part of the membership and which cost extra. Ask whether you would ever come to an office, or whether all care happens at home and by video. Ask how cancellation works and whether there is a minimum term.

Who is it a good fit for?

This is a good fit if you want one physician who handles everyday primary care and also manages hormones, weight medication and prevention, and you are comfortable with a membership and home or virtual visits. It is not the right fit if you want a single weight loss or hormone program with a posted price.
