What is Lavena Health?

Lavena Health is a physician-owned, cash-pay clinic at 5340 E 131st Ave, Suite 106, in Temple Terrace, Florida. Medical weight loss with GLP-1 medication is its main service, alongside hormone therapy, primary care memberships, vitality and longevity care, and Eastern medicine. It does not offer peptide therapy.

Dr. Adeeb Saleh, DO, is board-certified in emergency medicine and holds A4M certifications in hormone and weight-loss medicine. Dr. Ahmed Karim, MD, specializes in wellness, weight loss and longevity medicine. Dr. Chanelle Azar, DOM, is an NCCAOM board-certified acupuncturist and herbalist who leads its Eastern medicine care. In its own words: "Two physicians. Both owners. Both see patients."

Telehealth covers weight loss, hormone therapy and primary care for patients anywhere in Florida. Eastern medicine and hands-on treatments are done in person at the Temple Terrace clinic.

What does it cost?

Weight loss is one monthly price that includes medication and weekly visits. Semaglutide is $175 the first month, $200 a month for doses up to 1 mg a week, and $225 a month above that. Tirzepatide is $225 the first month, $275 a month up to 7.5 mg a week, and $350 a month above that. Labs are billed separately, and Lavena does not publish lab costs.

Hormone therapy is $150 a month for testosterone (TRT) or women's hormone therapy (BHRT), with labs separate. The ED program starts at $75 a month, vitality and longevity care runs from $175 to $225 a month, and hijama is $149 a session. Primary care memberships are $100 a month for one adult, $150 a month for a couple, and $150 a month plus $25 per additional child for a family. They run on a 12-month agreement billed monthly, and membership is not insurance. The clinic takes no insurance, and financing through Cherry is 0% APR for qualified patients.

What should you ask before you book?

Ask who signs your prescription. Dr. Saleh sets the GLP-1 protocols used at the clinic, so ask whether Dr. Saleh or another physician prescribes your dose and who reviews it each week.

Ask whether the medication is brand-name or compounded. Medication comes from US-based, licensed 503A pharmacies, so ask what that means for the product you would receive.

Ask what the monthly price leaves out. Labs are separate, and on memberships, procedures and program medications are separate too. Request a lab estimate before you start.

Ask about your state. Weight loss by telehealth is described across 40+ states, but patients outside Florida and Missouri are cared for through a partner physician network. Ask who would prescribe for you.

Ask how a membership ends. The 12-month agreement is billed monthly, so ask about cancellation and what happens if you want to leave early. Ask about office hours too, since none are listed.

Who is it a good fit for?

This is a good fit if you live in Florida, want cash-pay weight loss or hormone care with medication included in one monthly price, and will confirm the prescriber, pharmacy and lab costs before you start. It is not a fit if you need peptide therapy or insurance billing.
