What is Align Longevity Medicine?
Align Longevity Medicine is a physician-led practice at 2244 Faraday Ave., Suite 112, in Carlsbad, CA 92008. It is mainly a concierge practice, offering ongoing memberships, and it provides care in person in Carlsbad and by secure telehealth across California. Michael Arce, MD, Chief Medical Officer, directs the clinical care.
Its four areas are weight loss, hormone optimization, peptide therapy and longevity medicine. The weight loss program uses tirzepatide, and retatrutide is listed as coming soon. Hormone therapy can be given as pellets, oral or sublingual tablets, or injections. The peptide series is grouped by goal and includes products such as BPC-157, Semax and Epitalon. For concierge members, mobile services including blood draws and IV nutrient therapy are offered throughout San Diego County.
In its own words: "Physician-led. Team-delivered. Built around you."
What does it cost?
Two memberships have published monthly prices: $199 a month for the foundational plan and $350 a month for the plan focused on advanced optimization and ongoing partnership. Executive Care is offered by private consultation with no price shown, and no price is shown for the Shepherd or Guardian plans. It does not publish a consultation fee or prices for individual hormone, peptide or IV therapies.
The clinic does not say whether medication is included in any membership fee. It also does not say whether it accepts insurance, and it does not state its cancellation terms.
The $350 plan includes quarterly follow-up visits, two saliva evaluations a year and ongoing lab review. Ask which of those are inside the monthly fee and which are billed separately.
What should you ask before you book?
Ask who signs each prescription, and whether that is Dr. Arce or another clinician on the team. Ask whether the monthly fee covers tirzepatide, retatrutide or any peptide, or whether medication is billed on its own. Ask whether the tirzepatide is brand-name or compounded. The clinic itself states that compounded preparations are not FDA-approved, so the answer matters. Ask whether retatrutide is available yet, since it is listed as coming soon.
Ask what follow-up visits, lab work, saliva evaluations and mobile blood draws cost beyond a membership. Ask how to cancel, how much notice is required, and whether any charges continue after you stop. Ask whether any part of your care is billed to insurance.
Who is it a good fit for?
This is a good fit if you want an ongoing physician-directed membership and are ready to ask how medication is billed before you sign. If you want a single visit with a published price first, ask for the full fee schedule before booking, since none is posted for that kind of visit.
