What is Nuform Health?

Nuform Health is a telehealth practice with offices in Troy and Southfield, Michigan, built mainly around hormone therapy, compounded weight loss injections, peptide therapy and a short list of longevity treatments. It offers testosterone replacement therapy, an enclomiphene plan, DHEA and pregnenolone, a GLP-1 vitamin blend and a GLP+GIP vitamin blend for weight loss, Lipo-Mic B12, tesamorelin and sermorelin, rapamycin, NAD+, glutathione, diagnostic labs and a women's health track. Care runs entirely online: "Personalized care, 100% online." It describes itself as physician owned and operated, founded by Dr. Christopher Elia, DO, and Romario Yousif, PA-C, who it says still run the practice day to day, alongside Dr. Shawn Achtman, DO (physical medicine and rehab) and Megan Brias, RN. The weight loss medications are compounded, not brand-name semaglutide or tirzepatide, and are dispensed through U.S.-based 503A and 503B pharmacies.

What does it cost?

Joining costs a one-time $170 physician fee, which it says gives ongoing SMS and phone access to your care team. Testosterone therapy is sold as an injectable kit, dosed at 200mg weekly with supplies included, listed at $200 for four months ($50 a month) in one place and $120 a month for a one-month testosterone plus hCG or gonadorelin kit in another. The GLP Vitamin Blend runs $389 for a 3-month starter kit ($129 a month, includes supplies and dose increases) or $589 for a 5-month maintenance kit ($117 a month, supplies included). The GLP+GIP Vitamin Blend runs $499 for 2.5 months ($199 a month), $780 for 4.5 months ($173 a month), or $980 for 6.5 months ($150 a month). Lipo-Mic B12 is $360 for a 4-month kit ($90 a month). A separate page aimed at women lists the same two blends starting at $149 and $199 a month, with consultation billed apart from that. In its own words: "No. Each kit is a one-time payment covering its full length. Nothing recurs on your card and there is nothing to cancel." It does not accept insurance ("No insurance required") and notes HSA or FSA funds can be used.

What should you ask before you book?

Ask which prescriber reviews your labs and signs your prescription, since four names appear across the practice with different licenses. Ask whether the $170 fee or kit price covers your lab panel, since the site mentions testing over 57 biomarkers but prices are stated to exclude lab and consultation fees. Ask exactly what is in the GLP Vitamin Blend and GLP+GIP Vitamin Blend, since those are compounded formulas rather than a named FDA-approved drug. Ask whether the Troy or Southfield address is used for any in-person visit or lab draw, or whether everything, including blood work, happens remotely. Ask what happens at the end of a kit, since there is no subscription to cancel but a prescriber still has to approve each new cycle.

Who is it a good fit for?

This is a good fit for someone who wants a fully virtual, cash-pay relationship for hormone or weight loss treatment and is comfortable getting medication mailed in multi-month kits rather than month to month. It is a poorer fit for someone who wants an in-person exam, a brand-name GLP-1 prescription, or full price transparency before they ever speak to anyone.
