About Elevated Health

What is Elevated Health?

Elevated Health is a direct primary care and wellness practice at 5721 Fourth St. North in St. Petersburg, Florida. Its main business is membership-based primary care: you pay a monthly fee for a set number of visits and direct access to your provider. The membership tiers are Ivory, Black, Gold and Elite. Alongside primary care, it offers IV therapy, red light therapy, peptide therapy, medical weight management, hormone replacement therapy and cosmetic medicine.

The weight program uses GLP-1 medications with nutrition and exercise guidance, and it includes an InBody body composition scan. Hormone therapy begins with lab testing. The peptide list includes BPC-157, Ipamorelin, NAD+ and Low-Dose Naltrexone (LDN). Several peptides are marked as investigational and not FDA-approved for the uses described. Telehealth visits are offered, and the Black and Gold tiers include a set number of them. The practice names Tampa, Clearwater, Largo and several other towns in the Tampa Bay Area as places it serves. It does not name a provider who prescribes. It is rated 5 on Google from 28 reviews. In its own words, it is a "brand new practice," so ask how long it has been open.

This listing is for its St. Petersburg office at 5721 Fourth St. North.

What does it cost?

For medical weight management, non-members pay $299 a month in year one and $179 a month from year two. The year-one price covers an initial consult and seven follow-up appointments, and it includes labs and ongoing support. GLP-1 medication is billed separately, so your real monthly cost is higher than the listed price. Members pay $50 a month added to their membership, and Elite members get the program at no extra cost.

The monthly membership fee for each tier is not published. Memberships require a 12-month commitment, bill monthly, renew automatically, and have a minimum 60-day cancellation period. The practice does not accept insurance, but it accepts Health Savings Accounts for up to $150 of the monthly membership fee. Founders Circle memberships are offered at a discounted monthly rate for a limited time, and that rate is not shown. It does not publish prices for hormone therapy, peptide therapy, IV therapy, red light therapy or cosmetic medicine.

What should you ask before you book?

Start with who signs the prescription. Ask for the name and Florida license of the prescribing provider, and whether that person sees you in person or by telehealth. Ask whether the $299 or $179 price includes the GLP-1 medication, and what your total monthly bill is with it. Ask whether each medication is brand-name or compounded, and which pharmacy fills it. The weight page says prescriptions go through third-party pharmacies.

Then ask what follow-up visits and lab work cost after year one, and what hormone and peptide labs cost. Ask the monthly fee for each tier, and whether non-members can join one. Ask which states telehealth visits cover. Ask for the office hours, since none are listed. Finally, ask how cancellation works if you stop the weight program but keep the membership, given the 12-month commitment.

Who is it a good fit for?

This is a good fit if you want a membership-based primary care practice that also offers weight management or hormone therapy, and you are ready to pay a monthly membership plus a separate medication bill. It is a weaker fit if you need insurance billing or want every monthly cost in writing before you call.
