What is Springtide Medical?

Springtide Medical is a nurse practitioner-owned practice at 451 N. Maitland Ave., Maitland, FL 32751. Its main work is hormone optimization and medical weight loss, and it also lists telehealth services and peptide therapy. The prescriber is Melissa Collins, APRN, FNP-C, MSN.

Hormone optimization starts with a consultation and full-panel blood work. From there, the plan may include bioidentical hormone replacement therapy or hormone pellet therapy. The membership includes follow-up labs every 3 months, which it values at $150 to $200 per draw, along with same-day and priority visits, direct provider access and 10% off supplements through Fullscript. It does not say whether the medication itself is covered by the membership price.

The initial consultation and baseline labs are listed at $300 in its hormone pricing guide. A $250 consultation fee also appears in its material, so confirm which one applies. Annual full-panel labs cost $250 extra. Additional medications, such as compounded peptides or specialty injectables, are paid directly through its partner compounding pharmacy.

Pellet insertions are $350 for women and $700 for men at standard rates. Members pay $250 and $500 respectively. You can also pay per visit without a membership.

HSA, FSA and CareCredit are accepted. Membership and program fees are not covered by insurance, though some plans cover GLP-1 medications. It does not publish prices for the medical weight loss program, GLP-1 medication or peptide therapy, and it does not say what cancelling early costs.

What should you ask before you book?

Ask whether the $149 monthly fee includes any medication or only management and labs. If semaglutide or tirzepatide is prescribed, ask whether it is brand-name, what it costs each month and whether it is filled through the partner pharmacy. Ask whether any peptide you are considering is compounded and what it costs.

Ask which consultation fee applies, $300 or $250. Ask what follow-up visits and labs cost beyond the membership, and whether weight loss monitoring labs are extra. Ask how the 6-month agreement can be cancelled and what you owe if you stop early. If you plan to use telehealth, ask which labs or exams require an in-person visit in Maitland.
