About Evolving Vitality Clinic
What is Evolving Vitality Clinic?
Evolving Vitality Clinic is a telehealth practice with an office at 1444 S Belcher Rd, Ste C #2100, in Clearwater, Florida. It is U.S. veteran owned and operated, and it works mostly by telehealth. In its own words: "Evolving Vitality is a licensed Telehealth provider." It offers medical weight loss, hormone therapy and peptide therapy, along with personalized nutrition, vitamin and wellness injections, sexual wellness, and aesthetics and hair loss. The weight loss care includes tirzepatide, semaglutide, phentermine and metformin, plus peptides such as AOD-9604 and tesamorelin. Hormone plans use compounded testosterone cypionate with either enclomiphene or gonadorelin. Bloodwork is drawn at a LabCorp location. Office hours are 10:00 am to 5:00 pm Eastern, Monday through Friday, and closed on weekends. It serves patients in Florida, Utah, Colorado, New York and Wyoming. It does not name a prescribing clinician, and it does not say when it opened.
What does it cost?
The testosterone plan costs $349 for the first month, which covers the full lab panel, an onboarding consult and the first shipment. After that it is $199 a month, about $46 a week, and that price covers labs, automatic refills, supplies and provider oversight. Membership plans start at $149 a month for men and $129 a month for women, with no hidden fees and no contracts.
It does not publish prices for weight loss or peptide care, and it does not say whether medication is included in those plans. No consultation fee is given. The care is HSA and FSA eligible, and no insurance is required. It does not explain how cancellation works.
What should you ask before you book?
Ask who prescribes, and whether that person is licensed in your state. Its nutrition articles are written by a chiropractor, so ask directly who can write your prescriptions. Ask whether the monthly weight loss or peptide fee includes the medication, and whether each drug is brand-name or compounded. Ask which pharmacy fills your prescriptions. Ask whether products such as ECA Stack Plus and Lipo C injections need a prescription, and what they contain. Ask what follow-up visits and lab work cost after the first month, and whether any in-person visits are offered. Ask how to cancel, what notice is required, and whether any fee applies. Ask whether your state is one of the five it serves.
Who is it a good fit for?
This is a good fit if you want a telehealth practice with a Florida office, and you can get written answers about who prescribes, what the monthly fee covers and how to cancel before you pay. If you need in-person visits, or a price for weight loss or peptide care before you commit, call (800) 260-4992 first.
