What is E&O Wellness?
E&O Wellness is a concierge primary care practice at 12191 W 64th Ave STE 203 in Arvada, Colorado. Natalie Santucci, FNP, is the nurse practitioner who prescribes there and founded the practice. Primary care runs on a membership. The clinic also offers hormone replacement therapy with EvexiPEL pellets, GLP-1 medical weight loss, peptide therapy, platelet-rich plasma, neurotoxins and fillers, and microneedling.
Visits can be in person or by telehealth. The office is open Monday to Thursday, 9:00 to 4:00, and closed Friday through Sunday. The phone number is 970-805-0369, and it has a Google rating of 5 from 11 reviews. Membership is not limited to Arvada residents. Patients come from across the north Denver metro, including Wheat Ridge, Westminster, Lakewood and Golden.
What does it cost?
Membership sets the base price. Tier 1 is $110 a month for unlimited primary care. Tier 2 is $150 a month and adds a peptide catalog and discounts on HRT pellets, PRP and aesthetics. Tier 3 is $395 a month, and it is the only tier with GLP-1 weight loss care, with the medication included. Membership has a 3-month minimum, and cancelling takes 30 days' written notice.
Outside membership, services are priced one at a time. An initial HRT consultation is $125 for non-members. EvexiPEL pellet therapy is $425 for women and $775 for men without membership, or $375 and $700 with it. A PRP joint injection is $675 without membership and $475 with it. Dysport is $12 per unit, Xeomin is $10 per unit, and dermal filler is $550 per syringe. It does not publish peptide therapy prices.
On insurance, the clinic writes: "We do not bill insurance for office visits." It still uses your insurance for labs, imaging, prescriptions and specialist referrals. It does not say which insurance plans it accepts.
What should you ask before you book?
Ask who writes each prescription and who adjusts your plan between visits. Ask whether the GLP-1 medication is brand-name or compounded, because it does not say. Ask which lab work is covered by the $395 fee and what the rest costs, since labs are passed through at wholesale in the membership model.
Ask which peptides are offered, whether they are compounded, and what each one costs. Ask what testosterone therapy costs beyond the listed pellet prices. Ask what you would owe if you cancelled inside the 3-month minimum.
Who is it a good fit for?
This is a good fit if you want primary care, hormone care and possibly weight or peptide care under one membership, and you can commit to the 3-month minimum. It may not suit you if you need visits on Fridays or after 4:00 in the afternoon.
