About Surprise Care

What is Surprise Care?

Surprise Care is a primary care office at 12133 W. Bell Rd. Suite 101, Surprise, AZ 85378. The phone number is (623) 244-8797. It works on a Direct Primary Care membership, which its model is sometimes called concierge medicine, and it limits its patient list to no more than 500 patients. Its pages do not name the physician who prescribes.

Its services include minor illness and injury, annual checkups, medication management, vaccines, women's health, skin health, chronic disease management, sports and DOT physicals, COVID-19 and TB testing, and Emsculpt Neo body sculpting. Weight management is on the list too, but no weight-loss medication is named. Hormone therapy appears only as one item under chronic disease management.

Visits can be in person, by video, or by in-home appointment. In its own words: "Whether that is in-person at our clinic, through our telemedicine option online, through an in-home appointment, we have you covered." Google lists it at 4.6 from 45 reviews.

What does it cost?

Its main pages list the membership at $99 a month for adults, while its FAQ gives $69 a month. Ask which one applies. Children are $15 a month on a parent membership. Sports physicals are $25 and DOT physicals are $75. Financing is available through Cherry Payment Plans, including 0% APR plans, and through CareCredit.

There is no enrollment fee, but membership carries a 6-month commitment. The six months need not be paid up front, only the monthly fee. If you cancel early, charges are prorated to meet the six-month requirement.

It does not bill insurance for routine visits. Members can still use insurance for imaging, labs or medications outside the office when it applies. Members also get discounted lab rates and wholesale prices on commonly prescribed medications at its on-site pharmacy.

It does not publish a price for weight management or Emsculpt, and it does not say whether the membership fee covers medication.

What should you ask before you book?

Ask which weight-loss medications it prescribes and whether any of them are GLP-1 drugs. Ask whether it offers testosterone or hormone replacement, since hormone therapy appears only in a list of conditions. Ask who signs your prescriptions.

Ask whether any prescription is covered by the $99 or $69 fee, or whether medication is billed separately. Ask what lab work, imaging and follow-up cost. Ask how a early cancellation is prorated, and what its office hours are, since none are listed.

Who is it a good fit for?

This is a good fit if you want a local primary care doctor on a flat monthly membership and are willing to confirm the fee, the medication costs and the prescriber before you pay. If your main goal is weight-loss medication, call first and get the medication name and total monthly cost in writing before you enroll.
