Ask who writes the prescription for weight loss or testosterone, and what that clinician's credentials are. Ask whether the monthly fee includes the medication, and whether the drug is brand-name or compounded. Ask which weight loss medications the program uses, if any.
Ask what the initial evaluation, lab work and follow-up visits cost, and how often each is needed. For testosterone therapy, ask how monitoring is done and what it costs. Ask how you cancel and whether there is a contract. Ask whether any visits can be done by telehealth, and what the office hours are, since the clinic does not list them.
Who is it a good fit for?
This is a good fit if you want one Surprise office for primary care, pain treatment and a medical weight loss or hormone program, and you are willing to get prices and the prescriber's name in writing before you commit. If you need a published price list or a named prescriber before you call, wait until you have both.
