Side Effects: The Honest Version
TRT is generally well tolerated under proper monitoring, but there are effects every man should understand before starting:
- Testicular atrophy and fertility suppression. This is the big one clinics undersell. External testosterone signals your brain to stop stimulating your testes, which shrinks them and suppresses sperm production — sometimes to zero. If you may want children, say so before starting. Many clinics offer HCG alongside TRT to maintain testicular function, or enclomiphene as an alternative that raises testosterone while preserving fertility. Neither is a guarantee, but they exist for exactly this reason.
- Erythrocytosis (thickened blood). Testosterone stimulates red blood cell production. If hematocrit climbs too high, it raises clot risk. This is the single most common lab abnormality on TRT and the reason ongoing blood monitoring is non-negotiable. It's managed by dose reduction, switching formats, or blood donation.
- Acne and oily skin. Common, usually manageable.
- Fluid retention. Mild puffiness, especially early on.
- Prostate monitoring. Current evidence does not show TRT causes prostate cancer, but testosterone can accelerate an existing one — so baseline and periodic PSA checks are standard for men over 40.
- Sleep apnea can worsen in men who already have it.
